WHAT DOES IT FUND? The purpose of this program is to support and promote the development and expansion of dementia-capable home- and community-based service (HCBS) systems in states and communities. Click here for more information.

WHO'S ELIGIBLE? Domestic community based public and/or private home- and community-based service (HCBS) providers, including domestic public or private nonprofit entities, including state and local governments; Native American/Alaskan Native tribal governments and organizations; faith-based organizations; community-based organizations; hospitals; and institutions of higher education. Governmental entities within states and territories designated as state agencies for dementia capability are also eligible
