WHAT DOES IT FUND? This program provides comprehensive, coordinated, and evidenced-based services for individuals, youth, and families with a serious mental illness (SMI), serious emotional disturbance (SED), or co-occurring disorder (COD) who are experiencing homelessness, including encampments, or at imminent risk of homelessness, such as people exiting jail or prison without a place to live. States and territories, including the District of Columbia; political subdivisions of states; Indian tribes; tribal organizations; health facilities; and programs operated by, or in accordance with, a contract or award with the Indian Health Service or other public or private nonprofit entities.
