Grant Due: FY 2021 Integrated Networks to Deliver and Sustain Evidence-Based Falls Prevention Programs

WHAT DOES IT FUND? The purpose of this program is to maximize the independence, well-being, and mental health of older adults, people with disabilities, and their families and their caregivers by supporting projects that will strengthen integrated, sustainable networks that address the behavioral and social determinants of health of older adults and adults with disabilities, and increase the number of these individuals who participate in the evidence-based community programs to reduce falls and fall risk.
